    FDA License to Operate (Cosmetic Distributor) Initial Registration

    Assistance Fee: Php 15,000 

    FDA Payment via Landbank: Php 21,275 (3 years validity) 

    Requirements:

    • DTI (sole) / SEC (corp)
    • Mayor’s or Barangay Permit for Business
    • Government ID of the owner with 3 specimen signatures (passport, drivers, voters ID, SSS, GSIS)
    • PRC ID copy of Health Allied Personnel (Nursing, Med Tech, Radiology Tech, Pharmacist. Doctor of Medicine) with 3 specimen signatures
    • Notarized Manufacturing Contract

    Lead Time: 8-12 weeks

    FDA Cosmetic Product Notification (CPN) under your brand name 

    Registration Package Fee: ₱ 5,500 per PRODUCT (1 year Validity)
